NHS BOARD SLAMMED OVER BABY DEATHS

A 検死官 has criticised a health board に引き続いて an E.coli 突発/発生 at a hospital that killed two babies.

Five-day-old Hope Erin Evans and another child - referred to as Baby A1 - died after 契約ing a 特に deadly and 抗生物質-抵抗力のある 緊張する of the bacteria called ESBL.

An 検死 in Swansea heard Baby A1's mother was 診断するd with the bug after having IVF 治療 in India - and her 肯定的な 実験(する) result was 記録,記録的な/記録するd in a 医療の 公式文書,認めるs from abroad.

Hope, who was born at 26 weeks gestation, and Baby A1, born 27 weeks into her mother's pregnancy, were in incubators next to one another on Singleton Hospital's neo-誕生の 部隊 in November 2011.

The 審理,公聴会 was told their two コンテナs were only two metres apart - which is いっそう少なく than the three metre distance recommended by the British 協会 of Perinatal 薬/医学.

Although Hope 重さを計るd just 730 grammes and had been born 未熟に - she was still given a 75% chance of 生き残り by doctors.

But within 72 hours her 条件 quickly より悪くするd. She was pronounced dead when she was just five days old.

Hospital bosses at the Abertawe Bro Morgannwg health board said the 部隊 had since undergone a multimillion-続けざまに猛撃する refurbishment and "major 改良s" had been made. However, 事実上の/代理 Swansea 検死官 Colin Phillips said he would still be 令状ing up a 予防 Of その上の Deaths 報告(する)/憶測.

の中で the problems he identified was a 重要な hospital 文書 not asking medics to check whether their 患者s had so ught 治療 outside of the UK.

He said: "We have heard the E.coli was probably transmitted between the babies and that the mother of Baby A1 実験(する)d 肯定的な for E.coli ESBL 14 weeks in her pregnancy. This finding was 記録,記録的な/記録するd in her 医療の 公式文書,認めるs.

"E.coli ESBL 実験(する)ing is not routinely done on arrival to maternity 区s in this country, and 実験(する)s are only carried out when an 突発/発生 occurs.

"However, we have heard that the bacteria is ありふれた in places such as India 予定 to the over-use of 抗生物質s - and that there are pockets of the UK where 感染 is rising.

"I will be 令状ing a 予防 of 未来 Deaths 報告(する)/憶測 as I have 関心s important 患者 (警察などへの)密告,告訴(状) was not 逮捕(する)d."

Hope's 検死 also heard that had doctors discovered Mother A had E.coli they would have done things 異なって.

同様に as using 障壁 nursing, Mother A and her two twins would have been placed in a cubicle on their own.

Mr Phillips also 述べるd the 施設s at Singleton Hospital's neo-誕生の 部隊 before the refurbishment as "poor" and "outdated".

"The number of 沈むs were 不十分な for the number of cots and 列s formed at 頂点(に達する) periods," he 追加するd.

"The space between cots did not 会合,会う 現在の 基準s and a central (米)棚上げする/(英)提議する was used for 医療の 公式文書,認めるs.

"However, にもかかわらず these problems I have heard there was excellent 手渡す hygiene in place."

The 検死 also heard that 初めは Mother A had been 扱う/治療するd at Prince Charles Hospital in Merthyr Tydfil before 存在 transferred to Singleton after she went into 労働 早期に.

When 移転s like this occur, the 審理,公聴会 was told, an "の間の Hospital 移転 Documentation" should be 完全にするd.

When Mother A1 arrived at Singleton this form had not been 完全にするd.

However, Mr Phillips said if the 文書 had been 完全にするd it may have not made a difference because it did not ask the question about receiving 治療 abroad.

The 検死官 追加するd this was something which needed 演説(する)/住所ing - and 誘発するd him to 令状 a formal 報告(する)/憶測 on the 問題/発行する.

He said: "I am 関心d about the time lag and its been three years since Hope passed away and the 問題/発行する here is to 妨げる その上の deaths."

に引き続いて 調査s by Abertawe Bro Morgannwg University health board and 監視者 Healthcare Inspectorate むちの跡s, it still remains 不明瞭な 正確に/まさに when, where and how Hope (機の)カム to be 感染させるd.

In her 証拠 yesterday, 顧問 neo-natologist Dr ジーンズ Matthes said it was "not possible to 支配する out cross-感染 occurring".

The 審理,公聴会 was also told it was likely the "E.coli had been transmitted between the babies" - rather than coming from an outside source.

Mr Phillips 記録,記録的な/記録するd a narrative 判決, 明言する/公表するing Hope died from sepsis as a result of 契約ing E.coli ESBL.

Family solicitor Peter Williams said Hope's parents Jonathan and Janette, who live in Aberdare, were too upset to speak after the 審理,公聴会 had 結論するd.

Mother A's second child - referred to as Baby A2 - 生き残るd and was 発射する/解雇するd from hospital on December 29 2011.
