Co-op gave me my husband's ashes - but? the box had another man's 指名する inside:?SALLY SORTS IT

Last summer, my husband, from whom I was estranged, died suddenly. As his next of 肉親,親類, it was left to me to arrange his 火葬, which I did 経由で Co-op Funeralcare.

When I opened the box 含む/封じ込めるing the ashes on the day of the planned scattering, I 設立する paper with the 詳細(に述べる)s of a different man on it. The shock was indescribable.

Whose ashes did I have and what hope did I have of putting things 権利??Anon.

A reader was shocked when she found the wrong name in the box containing her husband's ashes ahead of the planned scattering

Sally Hamilton replies: The letter you sent me in 十分な 明らかにする/漏らすd the extent of the u pset you and your four grown-up children have 耐えるd に引き続いて your husband's death and the crass mistake over his ashes.

You told me how you had bumped into your husband for the first time in a while in town one day and agreed to 会合,会う for coffee. When he didn't show up, instinct told you something was wrong, and you went to his flat where you tragically 設立する him dead, 確認するd afterwards to be from natural 原因(となる)s.

As his next of 肉親,親類, you had to (疑いを)晴らす out the flat and を取り引きする 負債 需要・要求するs.

In まっただ中に this 強調する/ストレス, you arranged his 火葬. As he had little money, the Department for Work and 年金s agreed to 支払う/賃金 the 法案 of £1,195 to the 地元の 支店 of Co-op Funeralcare, with a 出資/貢献 of just £300 ― the balance left in your husband's bank account.

手はず/準備 were 延期するd as you were told by Co-op the 法案 had not been paid, while DWP 主張するd it had been. Weeks went by until Co-op 確認するd the 法案 had been paid and the 火葬 took place in September.

When you collected the ashes, they were supposed to be in a box inside a 捕らえる、獲得する labelled with your husband's 詳細(に述べる)s.

You made 計画(する)s to scatter his ashes at sea, as he wished. It was only on the day of the planned scattering that you opened the box to 除去する the 捕らえる、獲得する and discovered the paper with another man's 詳細(に述べる)s on.

After a string of calls and texts to Co-op, you asked for a letter 確認するing whose ashes you had. The 支店 経営者/支配人, you said, 辞退するd to 受託する 義務/負債 and requested you bring 支援する the ashes in person.

This 伴う/関わるd a 120-mile 一連の会議、交渉/完成する trip by car. Staff took the piece of paper with the other man's 詳細(に述べる)s and 知らせるd you the ashes you received were your husband's.

Unconvinced, you left, asking for this 確定/確認 to be put in 令状ing. Some time later, you received a letter ― unsigned and undated ― 申し込む/申し出ing £200 補償(金). You were furious and felt unable to scatter the ashes until you had more certainty.

By the time we spoke, you had 始めるd a (民事の)告訴 経由で the 国家の 協会 of Funeral Directors (NAFD), which runs an impartial (民事の)告訴s service. 一方/合間, I asked Co-op Funeralcare to give me its 味方する of the story. It said the ashes you received were certainly the 訂正する ones and that an 行政の 失敗 was to 非難する for the wrong piece of paper ending up inside the box.

A 広報担当者 said: 'We have 強健な 手続きs in place for the 身元確認,身分証明, care and return of ashes. A 徹底的な 調査 has been carried out, and from this we have 保証するd your reader that the ashes she received were those of her late husband'.

The '強健な 手続きs' 伴う/関わる the printing of a label from the 数字表示式の 記録,記録的な/記録するs of the 死んだ, which is 適用するd to the outside of the incinerator. After 火葬, this label is put on a biodegradable 捕らえる、獲得する 含む/封じ込めるing the ashes.

A second label 名簿(に載せる)/表(にあげる)ing the 死んだ's 指名する is then 適用するd to the box in which the 捕らえる、獲得する of ashes is placed. Finally, a 火葬 証明書 is placed inside the box ― which also says the 死んだ's 指名する. Co-op said it スピードを出す/記録につけるs ashes when they are received and a その上の label is 生成するd, which is placed inside the box and only 除去するd once the ashes are collected. It was at this 行う/開催する/段階 that things went horribly wrong in your 事例/患者, with a Co-op 従業員 挿入するing the wrong paperwork.

The 広報担当者 said: '予定 to an 行政の error, a label was incorrectly placed inside the box 含む/封じ込めるing her husband's ashes. We are 極端に sorry for the 苦しめる this has 原因(となる)d.'

にもかかわらず, Co-op 主張するd that the box of ashes 含む/封じ込めるd the three identifying labels with your husband's 指名する. It 追加するd that as you had requested the return of any metals from your husband's 団体/死体 and the 棺, these were 供給するd in a separate 捕らえる、獲得する in the box, 示すd with his 指名する and 火葬 number.

You remain unhappy, even after the findings of the NAFD 論争 決意/決議 service 受託するd Co-op's explanation. However, for the sake of your mental health, you have now decided, six months after your husband's death and に引き続いて my 介入, to settle. Co-op (機の)カム 支援する with an 申し込む/申し出 of £750 補償(金), which you have 受託するd. You are now planning to scatter the ashes.

I understand why you feel unsettled. There can be no 絶対の proof the ashes are his. デオキシリボ核酸 実験(する)ing would not 解決する your 疑問s, as it will have been destroyed by the heat of the 火葬.

Many (死が)奪い去るd families will be 関心d about this story, and, like me, hoping the funeral 産業 is doing the 最大限 possible to 避ける such mix-ups happening.

It may be small なぐさみ, but Co-op 確認するd that 're-training and education have been carried out to 確実にする all necessary lessons have been learned'.
