The 決まりきった仕事 操作／手術 that went horribly wrong

Kat Mclure was a woman with a passion for life. She had travelled, fallen in love, married on a Mediterranean island and, at 31, was the marketing director of a successful company.

But on November 22, 2005, during a 決まりきった仕事 操作／手術, she 苦しむd a heart attack and 壊滅的な brain 損失, leaving her to slowly waste away for six months before her death last May.

For her husband and family, the 不信 that this could happen to a fit and healthy young woman is made all the more 激しい by the knowledge that at the centre of the 悲劇 was the doctor ゆだねるd with her care.

Anaesthetist Ahmed el Sayed Moustafa has never 公然と explained what happened at the Belvedere Hospital in South London, where Kat was を受けるing 決まりきった仕事 plastic 外科 to alter the 形態／調整 of her nose and chin. Instead, it has been left to 捜査官／調査官s and 専門家s to try to piece together the terrible sequence of events.

Their 報告（する）／憶測s are universally damning. Dr Moustafa, a 69-year-old Egyptian who qualified 40 years ago in Cairo and has, he （人命などを）奪う，主張するs, carried out 47,000 操作／手術s in Britain since 1983 (which would mean he had carried out five 手続きs every day), failed to keep proper 公式文書，認めるs of the 麻薬s he gave Mrs McLure during the 操作／手術; used outdated resuscitation techniques known to have a high incidence of 失敗 and 複雑化s; and failed to support his 患者's breathing in an 救急車 にもかかわらず her struggle for breath. If this was shocking enough, what followed almost beggars belief.

Dr Moustafa was 一時停止するd by the Belvedere Hospital during its own 内部の 調査 and then banned from working there again. But he was not 妨げるd from working どこかよそで in the 私的な 部門.

As a freelance anaesthetist, he had 'practising 特権s' at a number of hospitals.

This meant he worked at the 私的な Portland Hospital in Central London until he was 一時停止するd by them in June 2006 for failing to tell them about the Belvedere 中断 and for forgetting to turn on the gas which would have kept a 患者 苦痛-解放する／自由な during 外科. Luckily, another member of the operating team noticed the mistake and told Dr Moustafa.

He also worked at the 私的な Highgate Hospital in North London, where 経営者／支配人s were 知らせるd about the Belvedere 出来事/事件 but continued to 雇う him after making their own 調査s.

Dr Moustafa was 一時停止するd by the hospital in December after the mother of Chelsea and England footballer John Terry had a heart attack on the operating （米）棚上げする／（英）提議する while in his care. 告訴する Terry 生き残るd and is 回復するing.

This 深く，強烈に troubling 目録 of 出来事/事件s over a short period is made all the worse by the knowledge that the General 医療の 会議 (GMC) - the 公式の／役人 団体／死体 責任がある 基準s の中で health professionals - was 知らせるd of Kat McLure's 事例／患者 a year ago and 明らかに did nothing.

It is only now, after a 検死官 asked the 主要都市の Police to 調査／捜査する her unexplained death and 告訴する Terry's 事例／患者, that the GMC has 一時停止するd Dr Moustafa's 権利 to practise and begun its own 調査.

Last week, it 拒絶する／低下するd to comment. But Dr Moustafa 主張するs he is blameless. At his home in the 豊富な Hampstead Garden 郊外 in North London, he said: 'There is nothing for me to say about this because I have done nothing wrong. I have done thousands of facials - 47,000 患者s in Britain ― and these are the only 出来事/事件s I have had happen. I should get a メダル. I am very 確信して I will be （疑いを）晴らすd by the GMC.

'I will not talk about the McLure 事例／患者 because there is an 調査 going on, but I am a 上級の doctor in my practice - I did 1,006 患者s after McLure - and this is beneath me. I am very sorry about the 患者 but these sorts of things happen.'

The anaesthetist, who is 存在 代表するd by his union, the 医療の Defence Union, went on to （人命などを）奪う，主張する that Mrs Terry, 48, had a heart 条件 before her 操作／手術, 追加するing: 'Anyway, she is alive.'

He says he will no longer pract ise as a doctor. 'I am 69, I have no 利益／興味 in continuing to practise.'

His 決定／判定勝ち（する） will bring little 慰安 to Kat McLure's husband, David, who is only beginning to come to 条件 with his loss.

The 35-year-old driver from Kettering in Northamptonshire takes anti-depressants and is planning to leave Britain to start afresh. It is only when he 会談 about his wife that he smiles. 'Kat had such a passion and 運動 for life,' he says. 'She had such 決意 to go and see and do. That's what made me 落ちる in love with her. We were planning our 未来 together.'

The only thing that worried Kat McLure was the 形態／調整 of her nose. Like thousands of others ― 2,268 in 2005 ― she decided to have plastic 外科.

'Kat had been talking about having a nose 職業 for a long time. It wasn't a whim. She had her dad's nose,' David says. 'She was beautiful to me, but she 手配中の，お尋ね者 to change its 形態／調整.'

She 研究d suitable 外科医s in Britain and chose Edward Latimer-Sayer, a Fellow of the 王室の College of 外科医s. She spoke to him on the telephone and then travelled to London for a 協議.

Two months later, she made an 任命 for the £5,200 手続き, which would take place in an operating theatre rented by Mr Latimer-Sayer at the Belvedere Hospital.

David says: 'It really galls me that she did all the 権利 things - 選ぶd a reputable 外科医 in this country - and this happened. There was never any 恐れる that something could go wrong. It was a simple 操作／手術 - a 45-minute 手続き and she was going to stay in 夜通し.'

But at 3pm on the day of the 操作／手術, David got a phone call from the Belvedere Hospital 説 Kat had been transferred to the Queen Elizabeth NHS Hospital in Woolwich, South London.

'They didn't give me any 詳細（に述べる）s and I thought it was just a small 複雑化,' he says. 'I phoned the other hospital and they asked how quickly I could get there. I couldn't get any sense out of the Belvedere.

'In one phone call I spoke to Mr Moustafa and he said it was nothing to do with him. It had never happened before. I was trying to 静める him 負かす／撃墜する, but he got so angry with me that I hung up.'

When David arrived at the Queen Elizabeth Hospital, his wife was in 集中的な care and was not 推定する／予想するd to last the night.

He says: 'The Queen Elizabeth doctors made it perfectly （疑いを）晴らす that something had gone horribly wrong. She was an 絶対の mess - she looked like she had been in a car 衝突，墜落.

'She had a 穴をあけるd 肺 and her 直面する looked terrible. The 外科医 had 現実に finished the 操作／手術 before 許すing her to be taken by the 救急車 and she was bleeding and one of her capped teeth was 行方不明の. She had 壊滅的な brain 損失. She didn't look like Kat any more.'

Kat, who was breathing 独立して, had been put into an induced 昏睡 as doctors at the Queen Elizabeth hospital waited to see if the swelling in her brain would stop.

'We had a bit of hope after 24 hours when she opened one 注目する，もくろむ,' David 解任するs, 'but that was the only 調印する she ever gave. I think that was it then.'

On December 13, David's birthday, the 顧問 neurologist told him that his wife would never 回復する. For the next six months, David and his wife's family stayed at Kat's 病人の枕元. 'We were just waiting. Her family felt as long as her heart was （警官の）巡回区域，受持ち区域ing there might be a 奇蹟, but she was just wasting away.

'I used to talk to her and wash her 直面する and rub moisturiser into her 手渡すs and feet because it was all I knew I could do. I felt I had failed her because I hadn't been there for her.'

David 始める，決める out to find out what had happened to his wife, 伴う／関わるing a 医療の 怠慢，過失 lawyer, Sarah Harman, and （売買）手数料，委託（する）／委員会／権限ing an 独立した・無所属 専門家 to review the 証拠. He discovered that the Queen Elizabeth Hospital and the Belvedere Hospital had already 報告（する）／憶測d the 出来事/事件 to the Healthcare （売買）手数料，委託（する）／委員会／権限 - the 医療の 監視者 - and the GMC.

While the GMC appeared to sit on its 手渡すs, the （売買）手数料，委託（する）／委員会／権限 carried out a review of the 事例／患者 and 配達するd a 報告（する）／憶測 last April, just days before Kat McLure's death, with strong 批評s of hospital 政策 and the 活動／戦闘s of Dr Moustafa.

The Belvedere had 設立する Dr Moustafa 有罪の of poor 記録，記録的な／記録する keeping and '不適切な 活動／戦闘s or omissions'.

The （売買）手数料，委託（する）／委員会／権限's 報告（する）／憶測 went その上の. It 設立する five 失敗s of 規則s 治める／統治するing 患者 care.

These were that the hospital did not 通知する them of the serious 傷害 to Mrs McLure (it was left to a third party to do this); there were no proper or '論理（学）の' 公式文書，認めるs kept on what happened during the 操作／手術 and some 医療の 記録，記録的な／記録するs were 行方不明の; the 同意 form for Mrs McLure was not 完全にするd '適切な' and there was no 証拠 of what Kat Mclure had been told about the 危険s of the 操作／手術; and Dr Moustafa had not undergone an 年次の review of his 技術s to 確実にする he was up to date with 手続きs and fit to practise.

And, most damning of all, the 報告（する）／憶測 設立する that there were no 始める，決める 手続きs for 取引，協定ing with cardiac 逮捕（する）s, 説: 'The 報告（する）／憶測d 治療 during the 緊急 状況／情勢 could not be seen to follow any other known methodology for 治療 of an 緊急 状況／情勢.'

It 追加するd that from the written 証拠 and 証言 利用できる to the （売買）手数料，委託（する）／委員会／権限 'it is （疑いを）晴らす that the practice 存在 undertaken at 逮捕（する） did not 会合，会う the UK Resuscitation 会議 指導基準s'.

This 含むd not using a defibrillator to shock the heart into （警官の）巡回区域，受持ち区域ing.

Instead, Dr Moustafa struck Mrs McLure's chest several times in an 試みる／企てる to 再開する her heart and then, after a 延期する, 治めるd adrenaline into her heart - a 手続き, によれば an 独立した・無所属 医療の 専門家, 'abandoned because of a high incidence of 失敗 and 複雑化s'.

The 欠如（する） of any coherent 公式文書，認めるs means that no one can say what 原因（となる）d Mrs McLure's heart attack.

The 独立した・無所属 専門家 協議するd by Mr McLure says her 血 圧力 dropped 危険に low and members of the 医療の staff 解任するd seeing Dr Moustafa 注入するing her with an 身元不明の 'スパイ／執行官', but no 結論s could be drawn on the 限られた／立憲的な （警察などへの）密告，告訴（状） 利用できる.

The Healthcare （売買）手数料，委託（する）／委員会／権限 says: 'It is （疑いを）晴らす that the 臨床の 行為／行う of the anaesthetist draws comment and 批評 from a number of ４半期/4分の1s.'

The matron of the Belvedere, Tina Fasholu, told the （売買）手数料，委託（する）／委員会／権限 that 'the culture within the hospital is such that hospital staff have difficulties in 介入するing when 態度s and personalities of some 医療の staff 無視／無効 their professional opinions'.

Richard Beldevere, 長，指導者 (n)役員／(a)執行力のある of the Belvedere, said last week: 'No one is able to comment on what 原因（となる）d the heart attack, but there was a 十分な grey area in the way the resuscitation was 扱うd to 一時停止する Dr Moustafa's practising 特権s and 通知する other hospitals that we had done so.'

Mr Tickle said he had to be careful about making 主張s against doctors because they are 'people with a lot of money who could 告訴する us over 廃虚ing their 評判 or costing them money in lost work', but he agreed the hospital's 義務 was to 保護する 患者s.

However, he 追加するd: 'Kat McLure was not our 患者. The 外科医 and his anaesthetist used our 施設s. Mr Moustafa had no 契約 with the hospital.'

The Healthcare （売買）手数料，委託（する）／委員会／権限 同意しないs and has ordered the hospital to review its 関係 with practitioners and to take '十分な 責任／義務' for governance 問題／発行するs at all times. It 追加するs: 'The practice of 患者s "belonging to the 顧問" is only true to a 確かな extent.'

The death of Kat McLure is now under the scrutiny of the 主要都市の Police Specialist 罪，犯罪 Directorate and her husband is planning a civil 活動／戦闘.

His solicitor, Sarah Harman said: 'What a lot of people believe is that they get a better 基準 of care in the 私的な 部門 than in the NHS, but this 事例／患者 論証するs that 私的な hospitals are under-規制するd and you can get a 基準 of care that is a whole lot worse.'

It is a 見解（をとる） that haunt s David McLure. 'I still have nightmares about what Kat must have gone through. She was in 苦痛 and 苦しめる and was alone. Things happen because nobody is perfect, but you ゆだねる your life to these people, so they should value that 責任／義務 enough to 収容する／認める it when things have gone wrong.'
