A fit and healthy woman bled to death after having 決まりきった仕事 surger y for a 支援する 苦痛.

Andrea Green, 42, died just 14 hours after the 操作／手術 at Barnsley 地区 General Hospital.

Her death （機の）カム after staff in the hospital's orthopaedic department 警告するd 経営者／支配人s about 墓／厳粛／彫る／重大な 危険s to 患者s 'extreme 圧力 and 強調する／ストレス' in the department.

An 検死 was 停止（させる）d and Sheffield 検死官 Chris Dorries asked police to 調査／捜査する に引き続いて the 出現 of the letter 輪郭（を描く）ing staff 関心s.

Ms Green had started 苦しむing from 支援する 苦痛 in August 2009. The 苦痛 was so bad that she was bedridden for two weeks にもかかわらず 存在 定める／命ずるd with painkillers by her GP on 非常に／多数の occasions.

She was referred to the orthopaedic clinic at Barnsley Hospital but by the time her 任命 （機の）カム 一連の会議、交渉／完成する, her 苦痛 had 沈下するd.

Andrew Harrison, 長，率いる of 医療の 怠慢，過失 at Raley's solicitors, which 代表するd the family, said serious 内部の bleeding after the 操作／手術 should have been spotted and 扱う／治療するd by doctors.

He said: 'They could have saved her for up to 30 minutes before she died . They had got all the （警察などへの）密告，告訴（状） if someone cared to look.'

The hospital was 警告するd of 'extreme 圧力 and 強調する／ストレス' in the orthopaedic department a letter just weeks before Andrea died in March, 2010.

A letter was sent to former 長，指導者 (n)役員／(a)執行力のある Sharon Taylor and referred to a 会合 between her and 医療の director Dr Jugnu Mahajan in which the orthopaedic team 警告するd of danger to 患者s.

The letter 明言する／公表するd 'it was only a 事柄 of time before the 状況／情勢 led to 重要な 患者 morbidity and mortality'.

The doctor who 成し遂げるd the 操作／手術 on Andrea is still working at the hospital under 監督.

A 広報担当者 for Barnsley 地区 General Hospital said: 'We are 極端に sorry for the loss 苦しむd by Andrea's family. We 扱う／治療する the safety of our 患者s as an 最大の 優先.

'After Andrea's death we began a 十分な 内部の 調査 and also sought the 見解（をとる）s of 外部の 専門家s.

'We have fully 器具／実施するd all of the changes recommended by those 調査s.

'The letter and its contents are a 事柄 for consideration at the 検死 and it would therefore be 不適切な for us to comment その上の.'
