SA dept questioned over woman's death
The carer of a 無能にするd Adelaide woman who died in horrific 条件s was 以前 banned from working with another provider, a 議会の 調査 has heard.
Ann Marie Smith's carer Rosa Maione was banned by Domiciliary Care in January 2013 after 恐らく stealing a （弁護士の）依頼人's money.
South Australia's 事実上の／代理 Department of Human Services 長，指導者 Lois Boswell told Monday's 審理，公聴会 she became aware of the 禁止（する） last week.
"There was no 明確な／細部 調査 and (it) was not 報告（する）／憶測d to the police because the family asked for it not to be, so it was an 内部の 禁止（する） within the department," Ms Boswell said.
The 調査 also heard Ms Maione was 認可するd for a 明言する／公表する 政府 disability 審査 check 18 days after Ms Smith's death on April 6.
The service provider, 正直さ Care SA, submitted and paid for the 使用／適用 on April 15.
"As soon as we were aware, we cancelled her 審査," Ms Boswell said.
"What is やめる 関心ing is that there was the 使用／適用 after Ann Marie's death because 正直さ Care had 義務s to 報告（する）／憶測 and advise us."
Ms Boswell said it was a "horrendous and 考えられない" idea that 正直さ Care SA 許すd only one person to be rostered for Ms Smith's care and was a 重要な failing.
"The circumstances of this are the first we've ever heard; where one person was the only one 供給するing care to someone who needed it seven days a week.
"Any responsible provider would know that doesn't appear to be possible.
"There has 明確に been 保護（する）／緊急輸入制限ing 失敗s for Ann Marie, and these may have occurred over a number of years."
Former 主要な／長／主犯 Community 訪問者 Maurice Corcoran said まっただ中に the 明言する／公表する 政府 計画／陰謀's 移行 to the NDIS, there were 関心s of people slipping through gaps and he was worried there were more 事例／患者s like Ms Smith's.
"I would have thought any 労働者 had to be 満足させるd with 審査 and any sort of history such as (Ms Maione's) would have 除去するd them from working with 攻撃を受けやすい people," he said.
"If we have these いわゆる 保護（する）／緊急輸入制限s and 審査s and people are still 許すd to work after a history of 乱用 in the 部門, then our systems are failing."
Ms Smith, 54, had cerebral palsy and died from serious illnesses that developed while receiving 十分な-time care in her Kensington Gardens home.
She was 認める to the 王室の Adelaide Hospital the previous day with 厳しい ulcerated and 感染させるd tissue and other serious illnesses after Ms Maione called an 救急車.
