NTSB finds Ride the Ducks 衝突,墜落 予定 to broken axle

SEATTLE (AP) ― The 国家の Transportation Safety Board 支配するd on Tuesday that the probable 原因(となる) of the 2015 Ride the Ducks 衝突,墜落 that killed five people and 負傷させるd 69 was a mechanical 失敗 and 妥当でない 維持/整備, 同様に as (法などの)抜け穴s in 連邦の oversight.

A 欠如(する) of 保護s for the 乗客s, 含むing seatbelts and "厳しく deformed" seats, 与える/捧げるd to the severity of the 衝突,墜落, NTSB Chairman Christopher Hart said.

The 水陸両性の 乗客 乗り物, or APV, was traveling north over the Aurora 橋(渡しをする) on 主要道路 99 in Seattle at about 40 mph when the axle broke and the driver lost 支配(する)/統制する. The 国家の Transportation Safety Board is 会合 Tuesday, Nov. 15, 2016, to 決定する the probable 原因(となる) of the 2015 衝突,墜落 of a Ride the Ducks 乗り物 that left five people dead and dozens 負傷させるd in Seattle. "APVs, or 'Duck Boats,' 供給する a unique sightseeing 選択 in many American cities," Hart said, "Yet until now, these 乗り物s have been operated without any regulatory oversight when driven on our roads and 主要道路s."

The board 非難するd Ride the Ducks International, which 製造(する)d the Duck 6 乗り物, for failing to 登録(する) as a 製造業者 with the 国家の 主要道路 Traffic Safety 行政, which would have 確実にするd better oversight. They also were 批判的な of Ride the Ducks Seattle for failing to 演説(する)/住所 a 公式発表 that 警告するd of problems with the axle 住宅s on the 乗り物s.

The board 問題/発行するd 10 new safety 推薦s that 含む 要求するing the use of seatbelts when the 乗り物s are on the road. They also 問題/発行するd one 緊急の 推薦 that 要求するs the company to order all of its franchises to stop using the stretch 乗り物s until their axle 住宅s can be 修理d or 取って代わるd.

示す Firmani, a 広報担当者 for Ride the Ducks Seattle, said the company has made a 名簿(に載せる)/表(にあげる) of vol untary changes to 操作/手術s since the 衝突,墜落.

They now only use the newer 見解/翻訳/版 of the Duck 乗り物, the トラックで運ぶ Duck, and stopped using the 欠陥のある "stretch Duck" models, he said. The also made safety 昇格s that 含む 360-degree ビデオs that are streamed live to (警察,軍隊などの)本部, he said.

The company 追加するd a second 乗組員 member to 許す the driver to 焦点(を合わせる) on the road, Firmani said.

Pat Buchanan, the company's lawyer, said the changes have 許すd them to receive the highest safety 率ing from the Washington 明言する/公表する 公共事業(料金)/有用性s and Transportation (売買)手数料,委託(する)/委員会/権限, which 監督するs 商業の 乗り物s. NTSB 報告(する)/憶測, he said, leaves much of the 非難する with the 製造業者.

"The 報告(する)/憶測 明確に 結論するs that Ride the Ducks International was a 乗り物 製造業者, 支配する to very 明確な/細部 支配するs and 規則s, 含むing 劇的な 高くする,増すd 責任/義務s for 警告 操作者s of safety 問題/発行するs," Buchanan said. "Had the company done its 義務 as 定める/命ずるd by the 規則s, we believe this 悲劇の 事故 would have never occurred."

In his 開始 声明, Hart said there were a chain of events over a period of years that led to the 衝突,墜落.

"Our 調査 設立する 行方不明の 層s of safety oversight in the way that APVs are 製造(する)d, 決定するd to be 安全な to operate, and 持続するd," he said.

Ride the Ducks International was aware of axle problems 支援する in 2004, and 試みる/企てるd to 直す/買収する,八百長をする it in 2005, but the modification "was 貧しく 遂行する/発効させるd."

Then in 2013, the company sent out a service 公式発表 警報ing 操作者s about problems with the axle 住宅 in the 乗り物s, 捜査官/調査官 Brian Bragonier told the board. After the 衝突,墜落, 捜査官/調査官s 設立する the 公式発表 in the Seattle company's office, but the work was never done, he said.

捜査官/調査官s 決定するd that Ride the Ducks Seattle 欠如(する)d the 議定書s needed to 確実にする that 維持/整備 was 完全にするd 適切に.

One of the board's 10 推薦s was for the Seattle company to 強化する its 手続きs to 確実にする all 活動/戦闘s in 公式発表s are 完全にするd.

Board member Robert Sumwalt pointed out that another 捜査官/調査官 設立する problems with the recommended 直す/買収する,八百長をする for the axle. To 演説(する)/住所 that, the board said the 製造業者 of these 乗り物s should 登録(する) with 連邦の regulators so defects are 演説(する)/住所d though a structured 修理 or 解任する 過程.

When the 衝突,墜落 happened, 11 people on the Duck Boat were 排除する/(飛行機などから)緊急脱出するd and seven of those people 支えるd serious 傷害s, said 捜査官/調査官 Ronald Kaminski.

任命する/導入するing seatbelts would 確実にする that 乗客s stay in the seating compartment while operating on land, the board said. But 操作者s will need to make sure that 乗客s 除去する the seatbelts when they are on the water, the board said.

"This 衝突,墜落 is a cautionary tale of what can happen when a 製造業者 does not follow 設立するd 支配するs about 直す/買収する,八百長をするing safety defects," Hart said.
