Patricia Fowler, 75, died after she was not seen by a hospital 顧問 for three days

A grandmother who was 認める to hospital with a broken arm died after she was left on a 区 for three days without seeing a 顧問.

Patricia Fowler, 75, was not 割り当てるd to a doctor at Blackpool Victoria Hospital, Lancashire, because she was overlooked by a 長官 who mistook her for another 患者, an 検死 heard.

There was 混乱 over who was caring for Mrs Fowler and she was left to grow ますます ill on a cardiac 区, even after her son raised 関心s over her health.

Mrs Fowler was 結局 seen by doctors three days after 存在 transferred to the 区 but by that point had developed 肺炎 and sepsis, which led to her death.

Son Scott Fowler said: 'やめる 簡単に, my mum died because she went into that hospital.?We all know that at some point we will lose our parents, but we 推定する／予想するd my mum - who was perfectly 井戸／弁護士席 - to be here for years and years.

'You don't die from a broken arm. You die from 肺炎 and sepsis and she got those because she was left on that 区 without doctors' reviews.

'She died on Wednesday 15 and I went into the hospital on the Tuesday night to see her - she looked terrible.

'I went up to the desk to tell someone and they just told me that she was tired and she'd be feeling better after a night's 残り／休憩（する）.

In a 声明 to the 検死官, Angela Russell, 医療の 長官 in the Care of the 年輩の department, 述べるd how Mrs Fowler was overlooked when she arrived.

She said 患者s' 指名するs were 株d on an email 名簿（に載せる）／表（にあげる） but there were no hospital numbers, NHS numbers or dates of birth.?

Ms Russell 追加するd: 'In this particular 事例／患者 there had been a 患者 with the same forename, which appeared in 正確に／まさに the same place on the emailed 名簿（に載せる）／表（にあげる）.

'When going through the 名簿（に載せる）／表（にあげる）, it was not obvious that the 患者's surname had changed.?I did not notice this and neither did any of the Care of the 年輩の 顧問s.'

Anaesthetist Dr Matthew Bowker told the 内部の 調査 how he had 表明するd 関心s after Mrs Fowler?wasn't 'reviewed by a member of her 医療の team' from the time she was moved の上に the cardiac 区 until three days later.

Another member of hospital staff, Matthew Bowker, told the 調査 there was '明白に no proper handover of care between 医療の 転換s', and said: 'It could be argued that had a proper timely review taken place, the severity of her 悪化／低下 could have been 妨げるd and she may have 避けるd admission to 集中的な care.'

Another medic, Dr Anju Mirakhur, also 述べるd the 混乱 over who was 責任がある Mrs Fowler's care.??

'A 十分な 調査 was carried out into the circumstances of Mrs Fowler's hospital 旅行 and a number of changes have been 器具／実施するd as a result of the findings of that 調査.'??
