A millionaire's wife hanged herself after arguing with her husband about her sleeping pill 中毒 - weeks after she 脅すd 自殺 on a 一連の会議、交渉/完成する-the-world trip.

Sharon Halliwell, 48, had just returned home after jetting off on a month-long holiday to?Singapore, Dubai and Malaysia with her husband, Darren Halliwell, 50.

The mother-of-two, from Wigan, Greater Manchester,?had a 長年の 中毒 to sleeping pills and ordered them in 本体,大部分/ばら積みの off the internet.?

She told her husband she had come off them and the pair went on holiday where Mrs Halliwell said she would kill herself on two separate occasions.

She had been 推定する/予想するing a referral to a psychiatrist upon her return to the UK but after a mistake by a mental health nurse, the 会合 was never made.

Because of the error at the North West Borough NHS 信用, she was instead seen by another community mental health team before her death on April 18 this year.?

Just two years before her death, her husband 恐らく 強襲,強姦d his wife onboard a British 航空路s flight to Houston, Texas.

His wife, who was also a grandmother, (人命などを)奪う,主張するd to have バリケードd herself in a bedroom and as a result he was 軍隊d to leave the family home in Aspull, Wigan, and given a three year 抑制するing order against his wife.

'Sharon enjoyed the gym and pampering 開会/開廷/会期s and was an 熱心な shopper. Throughout most of her life Sharon enjoyed 親族 good health.

'She did have problems with OCD but did 捜し出す 治療 for her 条件. She 苦しむd 苦悩 and 不景気 which became worse and then the girls left the family home.

'She had 問題/発行するs sleeping but didn't 捜し出す help from her GP and instead bought 医薬 from the internet.?

'She became (麻薬)常用者d to the sleeping tablets. We separated around 18 months ago, because of her 中毒.

'Our 関係 became volatile いつかs however we had been living together. After the 分離 Sharon moved to a new house on a new 広い地所.

'I do not have a 重要な for that house. I have a 重要な for the garage because I keep my motorbike in there and on occasions she would give me the 重要な to her house but would always ask for it 支援する.

'Sharon 認める that she was (麻薬)常用者d to taking the tablets. During this time she did some courses and had therapy to manage the 状況/情勢.

'In June 2016 she 受託するd that she was still taking the sleeping tablets and so we went to the 麻薬 and alcohol team.'

Mrs Halliwell had …に出席するd 任命s with the 麻薬 and alcohol team in Wigan and Leigh in 2016, and by November she was telling them that she was no longer relying on the sleeping pills.

Mr Halliwell 確認するd she was 申し込む/申し出d some support from them and was referred 支援する to her GP, but he had to …に出席する 任命s with her or she may not go. He also said that she seemed to be 改善するing.

Mrs Halliwell then began to 陳列する,発揮する suicidal thoughts and 活動/戦闘s, with Mr Halliwell 追加するing: 'She was talking more about having suicidal thoughts.

'She had talked about taking her own life before, that was nothing new. She did talk about it from time to time.

'We also decided to 調書をとる/予約する a holiday to Dubai, Singapore and Malaysia. Before this on three or four occasions Sharon made 脅しs to 害(を与える) herself.

'She said that she was going to tie something around her neck. On another occasion I did call the police as she left my home 説 she was going to 害(を与える) herself.

'I didn't really want to call the police because of our history, but I did call them and they got in 接触する with her and she said she was 罰金 and they said that she was not a 危険 to herself.

'We left for the holiday on February 24 and was away until March 20. On our return we believed that she was going to get an 任命 with a psychiatrist. She had never been 適切に 診断するd with an illness.

'On the first night of our holiday she said that she was going to take her own life by jumping off the roof in Singapore.

'The second day she tied a rope around her neck. I took it off her and she 静めるd 負かす/撃墜する.

'She was 罰金 for the 残り/休憩(する) of the holiday and she had gone 支援する to the Sharon I know.

'When we got 支援する she was having some work done to her house. I was around there and I 調印するd for a 小包. I 嫌疑者,容疑者/疑うd that it was sleeping pills.

'I asked her to open the 一括. She did and 認める it was sleeping pills. I told her that she should go to the doctor and they would 定める/命ずる them to her. That was the last time I was at the house.

'She had the work done to her home and stayed with me for a couple of days. On the Friday she was やめる argumentative and I don't know why she was just arguing about everything.

'It was about the tablets and she left and I didn't see her after that. She did text me a few different messages.

'She told me she loved me and then text me different messages, one was 説 she felt too sick to get up.

'I spoke to her on the phone in the evening. She didn't want me to tell anyone the 推論する/理由 that we weren't talking which was a 言及/関連 to her taking the sleeping tablets.

'I got a text from her on the Monday night 説 that she had managed to keep paracetamol 負かす/撃墜する and that she wouldn't be there in the morning.'

Mr Halliwell received a phone call from his daughter Levi Halliwell, 26, asking if he had heard from her mother later that day.

'Mrs Halliwell did not 現在の an 即座の 危険. She did say that in the past two weeks she had made 自殺 試みる/企てるs, but that she was not feeling that way at that moment in time.

'Her 試みる/企てるs were impulsive and she said that she did not want to die, and regretted her 活動/戦闘s after she had done them.

'We take into account the past and 現在の and 未来 and she said she was going on holiday and seemed to be looking 今後 to it.'

行方不明になる Woodcock also 認める that she could not 接近 triage 公式文書,認めるs made by an IAPT team, (改善するing 接近 to psychological therapies) on that day, but the 検死 was told that requests could be made for the informat イオン to be passed on to them.

Phillipa Killeen, the IAPT nurse who ?完全にするd the 審査 with Mrs Halliwell, told the 検死 that the Halliwells had both been 推定する/予想するing the 任命 to be one with a psychiatrist when she …に出席するd on April 3 this year.

Another 任命 was scheduled with the IAPT team for April 10 but Mrs Halliwell did not …に出席する, and so she was 接触するd and another 任命 直す/買収する,八百長をするd for May 8.

Ms Killeen 追加するd that Mrs Halliwell had 知らせるd her of her history of 自殺 試みる/企てるs, 含むing swallowing 殴打/砲列s, and the therapies that they could 申し込む/申し出 her were explained.

She said: 'She sent me a text explaining she wasn't 井戸/弁護士席 on the 10 April, so I called her and she said that she 手配中の,お尋ね者 to 調書をとる/予約する another 任命.

'We also discussed if she had spoken to her GP about an 査定/評価 and she said she had not, so before her next 任命 that was something I checked on and that was when I 設立する out that she had died.'

Ian Sturton-Crook, 長,率いる of 緊急の care at North West Borough Hospital, explained that changes had been made in their systems of communication since Mrs Halliwell death.

A 調査 報告(する)/憶測 showed that there were 'obvious gaps' in the communication between different systems, but also that the 失敗 to 言及する Mrs Halliwell had been taken into account and suitable changes made.

Mr Sturton-Crook said: 'The Rio system is one that (機の)カム into 影響 in November of last year and it is more 強健な than our previous systems.

'The mental health teams can 接近 the IAPT 公式文書,認めるs, but they have to ask for 接近 for them and in this 事例/患者 it was the 週末 and there was no 行政の staff to give 接近 to those 公式文書,認めるs.

'IAPT can 接近 the mental health t eam 公式文書,認めるs but it is not routinely done.'

He 追加するd: 'Everything was going 井戸/弁護士席. She was doing immaculately 井戸/弁護士席. She was 動機づけるd to reach her goals.

'She 手配中の,お尋ね者 to stop relying on the sleeping tablets and 手配中の,お尋ね者 to sort out the 関係 with Mr Halliwell. She said she loved him and 手配中の,お尋ね者 to be together with him.

'She felt it was her 中毒 to the sleeping tablets getting in the way of their 関係.

'She enjoyed going to the gym and often showed up in gym 着せる/賦与するs and we discussed how her gym going would have a 肯定的な 影響 on her mental health.

'The last 任命 she …に出席するd was on the 27 October 2016, where she said that she was 麻薬 解放する/自由な and feeling much better.

'She was put の上に the after care 計画(する), and was 招待するd to …に出席する group therapy and speak to the after care team, but she never did.

'We rely on co-操作/手術 with our (弁護士の)依頼人s in after care, we do not 軍隊 them to co-operate. It relies on their co-操作/手術.'

The 医療の 原因(となる) of death was given as 中断 by a ligature in the 地位,任命する mortem 報告(する)/憶測.

Area 検死官 Jennifer Leaming 記録,記録的な/記録するd a 結論 of 自殺, and explained that she would be advising a review of the communication between the services in the form of a 規則 28 review.

She said: 'It seems to me that さまざまな systems in place and the communication between them needs to be looked at with the North West Borough in particular.

'I think the system needs to be reviewed to ascertain what work needs to be done to 改善する the communication and for the the gap in the system can be の近くにd.

'I am very sorry for your loss, she has been 述べるd to be a a bubbly woman with an affinity for shopping and the gym, we have heard about how keen she was on that. I am very sorry.'

After the 検死, Mr Halliwell paid 尊敬の印 to his wife, 説: 'Sharon was a fun loving girly girl who treasured and was treasured by her friends and family.

'She was a much loved wife, daughter, mother and 充てるd nana who will be 大いに 行方不明になるd.

'The family are 完全に 荒廃させるd by her death and ask for privacy and time to grieve.'?
